The Opportunity:
Anglican Care have an exciting opportunity for an Enrolled Nurse to join the Home Care team in the Central Coast. As the Care Partner you will be responsible for assessing and coordinating care and support services for consumers assessing Commonwealth Home Support Services and Home Care Packages.
As well as providing clinical care you will also provide support, education and resources to consumers so they can make informed decisions and have choice and control over the services and support they receive and ensure the services delivered are safe, effective and meet consumers’ needs.

Role & Responsibilities: â¯
· Provide regular supervision with both clinical supervisors and line supervisors and deliver line supervision to clinical leads and other staff as needed
· Develop and maintain administrative systems, policies and procedures to support the efficient coordination of the programs and services and accurately record statistical data as required by Newcastle Anglican along with the provision of written plans, reports and documentation to support the delivery of services to clients.
· Develop and monitor processes for evaluating, reviewing and enhancing the programs and services to ensure that they remain cost effective and centred on the needs of clients
· Manage goals, budgets and expenditure reports for the programs and services. Prepare, in conjunction with the Finance Manager, annual service forecasts and budgets and manage the team to those budgets
· Assist in the development of strategic plans and ensuring they are effectively developed, monitored, evaluated and reviewed in line with the overall strategic goals of Newcastle Anglican. Reporting variances in the programs performance to the Director and Executive Director.
· Manage and oversee funded programs.
· Plan and initiate the marketing strategy for the programs and services and promotion of to encourage and prompt fundraising, grant, sponsorship opportunities for the programs and services and Newcastle Anglican.
· Oversee and manage the development of new initiatives and projects as required.
· Ensure that all government legislation, compliance requirements and the organisation's policies and procedures are implemented effectively.
· Monitor compliance with NDIS and funding agreements ensuring achievement of program targets, key performance indicators and quality standards
· Complete internal audits for services to ensure continuous improvement and high-quality service delivery
About you?
· Registration as an Enrolled Nurse
· Sound knowledge and experience working with people who are ageing from an enablement, restorative and wellbeing perspective
· Understanding of multidisciplinary teamwork and the ability to multi-task.
· Proven ability to conduct comprehensive assessments and develop goal orientated care plans
· Computer and technology skills to enable use of contemporary systems
· Skilled to engage and motivate consumers, families, staff and stakeholders.
